First Visit

We want you to have a great experience at your appointment! Here are some things to keep in mind before your visit:
 
Expect to be in the office for 2-2.5 hours for your first visit if you are starting treatment. This includes filling out paperwork and assessing your tooth by doing an exam and taking x-rays. Even though you were diagnosed elsewhere and referred for a root canal, Dr. Horcha does her own exam to confirm that a root canal is needed. If you would like an exam-only appointment please let us know when you call to schedule.
 
All adult patients 18 years old and over who are using insurance must bring their ID. Please also bring your insurance card and your referral sheet if the dentist who referred you if you have them. If you are pregnant, please contact your OB before the appointment and obtain a dental clearance form. They should have a standard form that they can send to you to bring to your visit.
 
If the patient is a minor, the parent or guardian MUST come to the appointment and may not leave the premises while the patient is being seen. Also, if the child is with a legal guardian instead of a parent, keep in mind that notarized guardianship paperwork from the state of Indiana is required. This is required by law for consent to be signed and we cannot make an exception.
 
If the tooth you need to be seen for has already had a root canal, please let us know before your appointment so that we can request the x-rays from your dentist.
 
When your root canal is completed, we will place a TEMPRORARY restoration. This is typical when a patient is referred to another doctor for the root canal. Please make sure to have the temporary replaced at your family dentist as soon as possible, preferably within 2-3 weeks.
